  • Schedule/Hours:
    • Monday - Thursday 9:00 am - 5:00 pm
    • Friday 9:00 am - 4:00 pm
  • Medical Setting: Outpatient
  • Call Requirements: None
  • Volume: 15-20
  • Patient Ages: All Ages
  • Procedures: Medication evaluation and management
  • Support Staff: LCSW, MA
  • EMR/Charting: Athena Net
  • Board Certification Requirements: Board Certified
